County governments were abolished in Franklin (July 1, 1997), Middlesex (July 11, 1997), Hampden and Worcester (July 1, 1998), Hampshire (January 1, 1999), Essex and Suffolk (July 1, 1999) and Berkshire (July 1, 2000). Barnstable, Bristol, Dukes, Nantucket, Norfolk and Plymouth kept theirs.
Barnstable County operates as the Cape Cod Regional Government under a home rule charter.
Suffolk County covers more than the city of Boston, but it is substantially consolidated with the city for governmental purposes.
Nantucket County has the same area and the same governing body as the town of Nantucket; the town's selectmen serve as county commissioners.

Why most Massachusetts counties have no government
On May 10, 1643, the Massachusetts Bay Colony divided its territory into four shires: Essex, Middlesex, Suffolk and an early Norfolk that was folded into Essex in 1680. Plymouth Colony created Barnstable, Bristol and Plymouth counties in 1685. Dukes County first appears in a 1683 New York law. The islands passed to Massachusetts under the 1692 charter, and in 1695 Martha's Vineyard and Nantucket were set up as separate counties. The other six are Hampshire (1662), Worcester (1731), Berkshire (1761), today's Norfolk (1793), Franklin (1811) and Hampden (1812). Residents mostly deal with town and state offices, not county ones. Chapter 48 of the Acts of 1997, which also had the state pay Middlesex County's debts, ended that county's government at once and scheduled Worcester and Hampden for 1998. In the abolished counties, sheriffs and registers of deeds are still elected, but state agencies manage them.
